Become a Patient of Dr. Jensen

Reach Dr. Jensen anytime, anyplace.

Dr. Jensen offers care through Direct Health (formerly the Medici app) a cutting-edge telehealth platform. It is HIPAA-compliant and allows for both audio/video appointments and 24/7 messaging. Telemedicine allows for the private and secure delivery of virtual healthcare. While he is no longer acting as a primary care provider, Dr. Jensen can prescribe supplements/medicine and order labs for you through the Medici app. He is currently licensed to provide care to patients in most states across America.

How to get started:

1. Download the Direct Health App, available for download on any smartphone. Be sure to download the app that says “Text Your Doctors” by the icon.

2. Create an account by entering the following information in the app:

Full name
Date of birth
Gender (as it appears on your birth certificate)
Phone number
Country

Agree to the terms and then OPT IN to receive text notifications so you can receive future communication from Dr. Jensen to you. Enter the activation code JENSQUE832 to find Dr. Jensen.

3. Finish your profile. Tap “PROFILE” in the bottom right. Tap “ADD PHARMACY” (skip if instructed to do so by staff member). Tap “PAYMENT” to arrange your preferred payment method. Tap “ADD NEW CARD” and fill out credit card info (even if you plan to pay with insurance). Tap “ADD,” and then tap “HOME” in the lower left and you should now be able to find Dr. Jensen’s profile on the home screen.

(This card will only be charged if you will not be using health insurance. If you plan to use insurance, follow the instructions in step 6.)

4. Find Dr. Scott M. Jensen’s profile by entering his address in the search bar: 6682 Lower Ridge Dr. Lakeside, AZ 85929 (His profile may be listed under either Lakeside or Pinetop-Lakeside. Both are correct.)

5. Send a message to Dr. Jensen with your new patient information. Once you have found his profile, TAP “MESSAGE” (under Dr. Jensen’s photo.) Your message MUST include the following information:

Reason for visit
Body weight of each patient
Medical history of the past 2 years (including past treatments/diagnoses in a hospital or doctors office)
Allergies, current conditions, chronic illnesses, etc.
Current medications/supplements each patient is taking
COVID test results and symptoms (if applicable)

Tap the arrow at the bottom of the page to send your message. You can then expect a reply through the Medici app regarding your care.

6. Send a picture/photocopy of your health insurance card to max.jfm@gmail.com if you will be using insurance to cover your visit.

We accept several PPO health insurances (Aetna, Cigna, BC/BS, UHC, Mercycare) but do not accept HMOs at this time.

Dr. Jensen’s rates begin at $117 for a single-person consult and $197 for more than one person (family rate). However, the consult rate may vary depending on the complexity of you or your family’s needs. Keep in mind that some (copay) or all of the cost of your visit may be covered by your insurance. Otherwise, the credit card connected to your Medici account will be billed.
